Spaceships & Surfboards: Cocoa Beach With Your Grandchild

After you enroll, we’ll ask you to provide or update your personal information and preferences, which includes your health information, lodging and meal preferences for your program.

A Parental Consent Form is required for minors and should be returned at least 5 weeks before the program start. This form must be signed by ALL custodial parent(s) / legal guardian(s) of the minor.  Parental Consent Form also MUST be completed in order for child to be permitted to participate. Please also complete and return the attached Waiver at the end of this packet to participate in our surfing lessons. This must be signed by a parent or guardian for participating children. Adults who wish to participate must also complete this form. Completed forms can be emailed to programadmin@roadscholar.org or mailed to Road Scholar, PO Box 56033, Boston, MA 02205. Please remember to fill out any and all health issues and/or dietary needs (adult and child), along with anything else we may need to know ahead of time so that we can better accommodate your stay with us! United States citizens don’t need a visa for any of the countries you’ll be visiting. Non-U.S. citizens should refer to their country's requirements to ensure they have the necessary documentation for travel.

A signed Parental Consent form is required for this program. The form must be signed by both parent(s)/legal guardian(s) of the child participating in the program and returned to the program provider prior to the start of the program. You will receive the form as part of your program information packet. You will also be able to access the form through your online account.
