Personal Information and Preferences
After you enroll, we’ll ask you to provide or update your personal information and preferences, which includes your health information, lodging and meal preferences for your program.
One or more of the meals on your program must be pre-selected. For your convenience, we have created an online form that can be filled out on any mobile device or computer. The link can be found in the Recommended Websites section of your pre-program information packet. We expect it will take less than 5 minutes to complete. At the bottom of the form there is a submit button. After submitting your form, you will have the opportunity to “submit another response” if you are traveling with someone in the same household.

Directions
From South to the Bayshore Resort
Take I-75 North to M-72 West to US-31 South, continue 5 miles to hotel on right.
From Traverse City's Cherry Capital Airport to the Bayshore Resort
Start heading west toward Wright Drive and keep left to take a slight right onto Fly Don't Drive. Turn left onto E South Airport Road and then in the next mile turn right on S Garfield Ave. Then take a left onto E Front St/US-31. The hotel will be on the right side. The total distance is 4.5. miles, about 15 minutes.
From North to the Bayshore Resort, Traverse City
Take I-75 South to M-72 West to US-31 South, continue 5 miles to hotel on right.
